A few months ago at my last job, they handed out a bomb threat checklist and asked us all to keep it near our phones. It was so silly that I figured it had to be an internal creation. Later, I googled "bomb threat checklist" and found that it's pretty much a standard form. Looks like it comes from ATF, even though they no longer host it online. I want all of you to print this form and keep it handy. If you don't have it, and receive a bomb threat, you may forget to ask the caller's name and address. They'll certainly give this information to you, so if you forget to ask, you're hampering the investigation.

2. "Where are you planning to use the checklist?"
"at . . . at my company?"

3. "What does your office building look like?"
"Wull. . . I dunno. Um, it's like, some office building in an industrial park here in Sunnyvale."

4. "What kind of office building is it?"
"I guess you could say it's kinda square."
"Did you say 'kinda square?'"
"Uh huh."
"Ok."

5. "What will you do with the checklist?"
"I'm giving it out to my coworkers."

6. "Will you distribute the checklist?"
"Yeah, like I just said."

7. "Why?"
"Cause my boss told me to."

8. "What is your building's address?"
"Why do you need to know my address?"
"So we can send you the form, sir. What is your building's address, sir?"
"Uh . . . wull ok. It's um. Let me look it up."
(Sound of paper shuffling)

"It's six twenty-four East Maude, Sunnyvale California, 95025."

9. "What is your name?"
"Joe."
"Joe what?"
"Joe Doe."
"Your name is Joe Doe."
"Yeah."
"Nobody's name is Joe Doe, sir. Please tell me your name."
"MY name is Joe Doe! It's Joe Doe!"
"Okay sir, I'm sending ATF 1613.1 Bomb Threat Checklist to Joe Doe at six twenty-four East Maude, Sunnyvale California, 95025."
"Okay thanks.
