I wonder if it should be removed first? I have other touchups I want to do elsewhere, I should take a real good look at all of them and see where they need it.

It's not worth removing unless it's unsafe or it's bothering you aesthetically. I've had moles inked over before - it's more tender but otherwise fine

It would be hard to say if that had irregular borders, wouldn't it? Maybe you should have a dermatologist look at it just to be safe.

I wonder if they'd have to do some other test instead. Scrape a bit off?

Yeah, if they thought it was concerning they would need to do a shave biopsy. Which is exactly what it sounds like. But if you're going to have the area re-tattooed anyway, who cares if they slice a bit of ink off?
